Virginia Davis Professional Coaching Coaching Services Agreement “Helping people navigate change with courage, clarity, and connection.” Client Information Full Legal Name Mailing Address Email Address Phone Number Description & Scope of Coaching Coaching is a collaborative partnership (defined as an alliance, not a legal business partnership) designed to facilitate personal, professional, or career goals through a creative and thought-provoking process. Standards of Ethics: The Coach abides by the Code of Ethics established by the International Coaching Federation (ICF). Client Ownership & Non-Therapy Notice: The Client retains sole responsibility for their decisions, actions, and results. Coaching is not psychotherapy, mental health treatment, legal counsel, or medical advice. Engagement: The Client agrees to communicate openly, bring self-honesty to sessions, and allocate the necessary time and focus to support their development. Services, Fees & Cancellation Session Fee: Coaching is provided on a pro-bono basis (regular professional rate: $65.00 CAD per hour). Meeting Format: Sessions are conducted virtually via secure Zoom video or phone. Cancellation Policy: A minimum of 48 hours’ notice is required to cancel or reschedule a session. Refund Policy: As this session is provided on a pro-bono basis, no financial fees are billed or collected. Confidentiality & Record Retention All coaching conversations and client materials are strictly confidential under the ICF Code of Ethics. Confidentiality is subject to standard legal exceptions, including statutory obligations, court orders, imminent risk of harm to self or others, or child protection reporting duties. In accordance with practice standards, client records and documentation are maintained securely in digital storage for a period of 10 years before being permanently destroyed. ICF Credentialing Verification Consent To maintain credentials and professional standing, the Coach submits an hours verification log to the International Coaching Federation (ICF). Only your name, contact details, and dates of service are shared solely for audit verification; no coaching topics or notes are ever disclosed.
