Coaching Confidentiality & Credentialing Consent Agreement

Introduction & Purpose

As your Coach, I am devoted to your success and committed to providing services that support your personal and professional growth. I aim to offer coaching based on a foundation of confidentiality, ethics, respect, reliability, and professionalism.

To maintain high standards of excellence and continuous professional development, I am actively pursuing credentialing through the International Coaching Federation (ICF). In alignment with this process, I respectfully request your permission for the following items:

ICF Credentialing Permissions

1. Coaching Hours Log

Permission to include your name and email address in a log of coaching hours submitted as part of my ICF credentialing application. An ICF credentialing assessor may contact you solely to verify that coaching took place. All session details and content remain strictly confidential; no other personal information will be shared.

2. Session Recording for Mentorship

Permission to share an audio or video recording of a coaching session with Susan Britton, MCC, an ICF mentor coach, strictly for professional development, feedback, and mentoring.

3. Session Recording for Accreditation

Permission to submit a recording of a coaching session directly to the International Coaching Federation (ICF) to fulfill credentialing performance evaluation requirements.

Core Coaching Principles & Boundaries

  • Confidentiality: All coaching conversations, materials, and records are held in strict confidence and will not be shared without your prior knowledge and written consent. Standard legal exceptions apply, including situations where disclosure is required by law, where there is imminent threat of harm to yourself or others, or cases involving suspected abuse.
  • Standards of Ethical Conduct: I strictly abide by the International Coaching Federation (ICF) Code of Ethics.
  • Nature of the Relationship: Coaching is a collaborative partnership focused on professional and personal goals. It is distinct from psychological counseling, mental health care, psychotherapy, or medical advice. Coaching outcomes are not guaranteed, and you remain fully responsible for your own choices, decisions, actions, and results.
